Required Documents for Student enrollment:

 

  • transcript (grades 9-12)
  • report card (most recent)
  • birth certificate (or other form of identity. See below)
  • social security card or S number
  • STAAR scores- 3rd graders and up (most recent) can get results here: https://texasassessment.com/families/ 
  • parents/guardians driver license.
  • proof of residency -

                       FOR STAFF ONLY: Parents have up to 30 days after they start to provide  the                     proof of residency


Affidavit of Student Residency

Verification of Residency (Texas Education Code 25.001(c)): Please provide at least two (2) of the following documents in Parent's/Guardian's name to show proof of residency of the address indicated in this Affidavit of Student Residency. A current , valid Texas Driver’s License with the correct address qualifies as one of the two required documents and is a preferred method of proof:

Current vehicle insurance or registration
